The main contribution to failure of any system is human. The human caused failures are due to following reasons:-
- Poor workmanship – during manufacture of electrical equipments, transit(while moving of carrying from one place to another) , during installation of equimpents and due to incorrect operations.
- Lack of experience, knowledge & willingness towards work.
- Lacking consistency of self-improvement.
The other non-technical reasons behind failures may be:-
- Deficiency of capital.
- Lack of time – production target to be achieved
The technical reasons behind failures of system:-
- Wear and tear of rotating, moving parts
- Atmospheric affects (Moisture, dust etc.)
- Aging of parts in load
- Long shutdowns
- Unpredicted large change in load
- Unpredicted large change in supply
- Failure of any other equipment in same supply
- No periodic maintenance
- Loose connections
